27-Apr-04: Inquest

Summary:

He was 61 years-old at DFD, he was killed on the work site by a steel pipe that he and another worker were working on. Henry cut the pipe so it could be pushed over by the other worker. The pipe fell and bounced back and hit Henry in the midsection causing his death. The pipe was 18 feet long and weighted about 1,000 pounds. It was not secured at the top for lowering pipe slowly to the ground before Henry began cutting it and it was not secured at the bottom to prevent kick back once toppled.

An inquest was held into his death in hopes to prevent any other deaths.
